Seek a Second Opinion if You:
- Need confirmation of your diagnosis and the extent of your disease.
- Have questions about your diagnosis, current course of treatment, or therapy options.
- Require medical information delivered in your preferred language.
- Would like information about clinical trials that are appropriate for you.
To Be Eligible for a Free Second Opinion:
- An adult living and receiving medical care in California
- You have received a cancer diagnosis and treatment plan, currently receiving treatment or finished treatment and looking for additional treatment options.
- You are able to attend a virtual panel either by video or phone.
- Completed all ‘Patient Intake Forms’ and submitted them as instructed.
- You or your representative have contacted our office (415-775-9956) to start the process.
What We Will Do for You:
- Collect your medical records, imaging and pathology from your medical providers.
- Have a multi-disciplinary panel of volunteer doctors review your records, images and pathology.
- Arrange a Zoom meeting for you and your family with our team of physicians.
- Provide you with an overview of your diagnosis and treatment plan, and answer all of your questions.
- Supply a written summary of the panel review to both you and at your request your treating physicians.
What Happens Next:
- Your case will be reviewed in 1-7 days after receipt by the Medical Director to determine how best we can assist you.
- Staff will contact you if they need additional information or to set up a time/date for a second opinion consultation.

How to Apply for a Free Second Opinion:

There are two ways to apply. You may use our HIPPA compliant online form, or you may save our PDF form on your computer to complete and email or mail it to us as listed below.
Apply Online:
Fill out the “New Patient Form” through a HIPPA compliant online form.
Make sure to click on ‘submit’ once you complete the form.
